A Clinical Study on Neonatal Seizures

Abstract
From December. 1990 to May. 1994. 31 cases of of Neonatal seizures who were admitted to Department of Pediatrics, Wonkwang University School of Medicine were clinically investigated and obtained following results.1 . The most frequent cause of neonatal seizure was hypoxic ischemic encephalo- pathy(45.7%), and followed by hypocalcemia(17.1%), intraventricular hemorrhage(11.4%), sepsis(8.6%), and hypoglycemia and intracerebral hemorrhage (3.2%, respectively), in order of frequency.2. The time of onset of neonatal seizure was within 24 hours of postnatal age in 11cases of 31 patients(31.4%). 2nd. day in 25.7%. and 3rd. day in 8.6%. 77.7% of neontal seizure occurred within 7 days of postnatal age.3. The most frequent type of neontal seizure was subtle(29.1%), and followed by multifocal clonic(25.8%), focal clonic(22.6%), generalized tonic(16.1%), and focal tonic and myoclonic(3.2%, respectively), in order of frequency.4 The brain computed tomography(CT) was performed in 24 cases. Abnormal CT scans were found in 54.1% of that patients and were diffuse cerebral isschemia in 5 cases, focal cerebral ischemia in 3 cases, cortical hemorrhage.Dandy-Walker cyst, and hydrocephalus in 1 case, respectively. The brain magnetic resonance imaging(MRI) was done in 5 patients. 3 patients had abnormal findings: Intraventricular hemorrhage in 2 cases, status marmoratus in 1 case.5. The Electroencephalography(EEG) was performed in 22 cases, and showed abnormal findings in 40.9%. Multifocal clonic seizures were associated with abnormal EEG in 80%, subtle seizures in 37.5%. and focal clonic seizures in 25%.6. The follow-up examinations were made on 24 patients(77.4%) during the period from 5 days to 26 months. 8 cases(33.3%) showed neurologic sequelae which were recurrent seizure in 5 cases(20.8%), cerebral palsy in 3 cases( 12.5%). and 2 cases expired.
